If you are not employed or otherwise compensated by the hospice company for your services as an attending physician, and have been identified as the attending physician by the patient (important!) you should use the GV modifier for services that are related to treatment of the patient's terminal condition i.e. the specific diagnosis with which they were enrolled in hospice.
If the service provided was not directly related to their hospice- eligible diagnosis, the GW modifier should be used.
